Home and Community-Based Services
Services | Requirements | How to Enroll

The Home and Community-Based Services (HCS) program provides community based services and supports to eligible persons as an alternative to the Intermediate Care Facilities for Persons with Mental Retardation (ICF/MR) program. Services are available to persons living in their own home and living with their family or paid companion (foster care provider). Services may also be provided to persons living in small group homes where no more than four persons may live.

UCP of North Texas HCS program provides services to individuals living in Dallas, Tarrant, Collin, Denton, Hopkins, Camp, Delta, Ellis, Franklin, Hunt, Lamar, Kaufman, Morris, Navarro, Rockwall, Titus, and Johnson counties in North Texas. Our priority is to learn what our consumers need, and then search the community for the best programs to suite those needs. Qualified trained professionals, such as Case Managers, Occupational, Physical and Speech/Language Therapist, Audiologist, Psychologist, Nurses and Dietitians and the consumer and/or their families form a team called an Interdisciplinary Team. This team develops an Individual Service Plan for the consumer revealing their needs including, but not limited to, housing, employment, and medical care. The team will work together to determine what is best for the consumer and the consumer and/or family representative are active members of the team. The ultimate goal of the UCP of North Texas HCS program is “To provide our consumers with the highest quality programs so they may learn and develop their skills to become contributing members of the community.”

    • Department of Assistive & Rehabilitation Services that help consumers find competitive employment in the community


Eligibility Requirements for HCS (back)

  • No age limit

  • Have had a determination of Mental Retardation(Click Here) made in accordance with state law or have been diagnosed by a physician as having a related condition.

  • The term "related condition" means a severe, chronic disability that meets all of the following conditions:

    • a condition attributable to:
      (i) cerebral palsy or epilepsy; or
      (ii) any other condition including autism, but excluding mental illness, found to be closely related to mental retardation because this condition results in impairment of general intellectual functioning or adaptive behavior similar to that of persons with mental retardation and requires treatment or services similar to those required for these persons;

    • a condition manifested before the person reaches age 22 years;

    • a condition likely to continue indefinitely
